Police said the car was travelling north on Mortlake-Ararat Road, near Denicull Creek, about 3pm when it crashed into a tree.
The driver, 65, died at the scene.
Police will prepare a report for the coroner.
Ambulance Victoria spokesman John Mullen said a boy, 11, had the most serious injuries of the five survivors.
"He had an altered state of consciousness and an injured leg," he said.
The four other children, believed to be aged between five and 12, had a range of minor injuries.
Paramedics transported the children to Ararat and District Hospital.
Ararat police appealed for the driver of a four-wheel-drive who stopped to help before police arrived to call the station on 53551500 or Crime Stoppers on 1800333000.
