Victoria Police Senior Constable Melissa Seach said two cars collided on the Western Highway at 1pm on Sunday.
A male driver and his female passenger were trapped in one car.
Sen Const Seach said the woman died at the scene.
She said the man was airlifted to Melbourne's The Alfred hospital in a critical condition.
"The female driver of the second vehicle has been taken to Stawell hospital for observation,'' she said.
"Two children, who were passengers, received minor injuries and have been taken to Royal Children's Hospital."
Stawell Leading Senior Constable Jo Orr said emergency services worked to free the trapped occupants.
She said the man suffered head injuries.
Leading Sen Const Orr said the Western Highway remained closed near the scene - two kilometres west of Stawell.
She declined to identify the people involved.
Sen Const Seach said police were investigating the circumstances surrounding the collision.
She said any witnesses should call Crime Stoppers on 1800 333 000.
