Roo Tom Magee received the injury in Saturday's senior match against Horsham Saints at Coughlin Park.
Western Vic Football manager Stephen McQueen confirmed Dimboola had requested an investigation.
"There is an investigation underway into an incident in that game," he said.
"The investigation officer has 21 days to look into it and present their findings to the league."
Dimboola president Rob Pyers said Magee had surgery on his cheekbone which was fractured in three places earlier this week.
"Tom sustained a pretty significant facial injury and we just want to get some clarity about how that happened," he said.
"We've made a request for an incident to be investigated.
"We're not making any accusations we just want some clarity about the incident and to let due process take its course. Tom's long-term health and well-being is most important from our point of view."
Horsham Saints president Roger Sordello said his club would fully support the investigation.
"We have a great relationship with Dimboola and I understand the club's concern," he said.
"The investigation will go through its normal process and we'll support the league and the investigating officer as much as possible.
"From a club perspective we wish Tom a fast and speedy recovery."
