St Vincent's Hospital neurosurgeon Paul Smith, from Melbourne, will consult from the Wimmera Medical Centre once a month.
He said he decided to set up an office in Horsham after he released there weren't any similar services in the region.
"Other services are a long way away, and seeing Horsham has a large population there was an opportunity to expand to the region," he said.
He said the service would also attract customers from South Australia.
"I like going to the country - I own a few farms and I like cropping country," he said.
"I grew up on a farm near Echuca - it was a sheep grazing property."
Dr Smith's office was open for the first time on Friday.
"It was a great day and people were happy I was there," he said.
"I was surprised, some people still travelled 80 kilometres or more to come to Horsham.
"I hear patients in the city complain about travel but it is nothing compared to living in the country."
Dr Smith said he could assess patients and arrange imaging, such as MRIs.
"Imgaging can be done in Horsham as well," he said.
"If a patient needs surgery, they will have to come to Melbourne but most of the time people who come to see me don't need surgery.
"What they need is advice and sometimes further imaging.
"Not having surgery will save people have to do a trip into the city."
Dr Smith said he would be in Horsham for a full day once a month on a Friday.
He said he became interested in neurosurgery while at university.
"I went to Melbourne University after finishing year 12 at Echuca," he said.
"I studied a medical course and become interested in neurosurgery while on rotation with a junior doctor.
"You sometimes just fall into these things."
Dr Smith said he was looking forward to visiting the Wimmera each month.
"It's something a bit different and I don't mind the drive," he said.
"Horsham is a great town and I love it.
"It's nice driving through, although in some parts the crops looked like they could use a bit more rain."

"People can also fill in forms from the website about their health issues before they come and see me."
