Hill was in a group of horses on the final turn when his horse The Muckle Toon stumbled.
Hill fell from the horse, which rolled on him.
Ambulance Victoria spokesman John Mullen said Hill was in a critical condition.
He said Ambulance Victoria received the call-out at 3.12pm, and the air ambulance landed at the scene at 4pm.
Hill has been flown to The Alfred hospital in Melbourne.
Wimmera trainer Terry O'Sullivan said Hill was meant to race for him in the final race of the day.
"It is bloody distressing and really sad and we just hope and pray he is okay," he said.
"Jack unfortunately clipped heels on the corner, the horse had a horrendous fall, and I think they've put him into an induced coma.
"I've seen a few fairly nasty falls, and thankfully it is uncommon."
The rest of the races have been abandoned.
