Horsham Acting Sergeant Heath Martin said the man, 72, was driving south on Lower Norton-Nurrabiel Road at Nurrabiel just after 2pm when his vehicle veered off the road into a tree.
He said the man was taken by road ambulance to Horsham Aerodrome.
He was then taken by air ambulance to Melbourne with internal injuries.
Act Sgt Martin said the man was the car's only occupant.
The road was blocked for about an hour as Horsham State Emergency Service and Wonwondah Fire Brigade crews worked to clear the tree.
Act Sgt Martin said police would investigate whether fatigue was a factor in the incident.
