The woman sustained serious injuries after the Holden Commodore she was travelling in collided with a truck south of Sisters Rocks about 12.45pm on Wednesday.
Victoria Police media officer Adam West said the woman was taken to Stawell Hospital in a critical condition and was later airlifted to The Alfred in Melbourne.
She died later that night.
Sen Const West said the woman, in her 20s, was travelling west and lost control of her car, which crossed onto the wrong side of the road and collided with an east-bound truck.
The truck driver is helping police with enquiries.

THE Western Highway between Stawell and Ararat has reopened after a crash.
The highway was closed for about five hours after a car and a truck collided on the Western Highway near the Black Range.
A woman, an occupant in the car, is in Stawell Hospital in a serious condition.
Police are investigating the incident.
Wednesday – 1.15pm
A WOMAN is in a serious condition after the car she was travelling in and a small truck collided on the Western Highway near the Black Range.
CFA, SES, ambulance and police officers attended the scene.
CFA District 16 operations officer Ian Morley said the crash happened about 12.45pm on Wednesday on the railway overpass south of Sisters Rocks.
“One lady has been removed from the car and is in a serious condition,” he said.
“Ambulance officers are providing first aid, and will take the woman by road to Stawell Hospital.”
Mr Morley said CFA crews stayed on scene while the road was cleared.
The Western Highway is closed in the area.
The highway is closed from the Western Highway in Ararat to Seaby Street in Stawell.
Detours are in place at Ararat-Halls Gap Road and Pomonal Road.
The highway is due to reopen about 6pm.
