Grampians Pyrenees Primary Care Partnership and Grampians Integrated Cancer Services have developed a online service directory for Wimmera resident and health professionals. 

Project co-ordinator Anna Greene said the service guide draws live data from the National Health Service Directory for more than 1200 groups from the South Australian border to Bacchus Marsh. 

She said health professionals and the general public could access essential service and support  information close to their own location.

“The experience of patients is greatly improved when they have reliable and accurate information easily accessible,” Mrs Greene said.

“For health practitioners, the directory helps to facilitate the coordination of ongoing care for their patients, providing connection of care and referral pathways for location based services.”

Mrs Greene said residents could access the directory on the Grampians Integrated Cancer Services website or get more information by calling 5352 6225.
