Nathan Cameron, 39, was allegedly driving a Holden Commodore when he swerved off the Peninsula Link freeway at Carrum Downs and struck the tow truck driver who was helping another motorist in the emergency lane on November 14.
The impact caused the Commodore to flip and land on its roof several metres along the freeway.
The tow truck driver was hit while loading a four-wheel drive that had broken down on the side of the freeway, police say.
The driver of the 4WD suffered minor injuries after being hit by debris.
The tow truck driver, aged in his 30s, had his arm severed, reportedly from the elbow down.
He underwent surgery at The Alfred hospital, but attempts to reattach his arm were reportedly unsuccessful. He remains in hospital in a stable condition.
Mr Cameron faces five charges, including dangerous driving causing serious injury and driving in an emergency stopping lane.
He briefly fronted Melbourne Magistrates Court on Friday and his bail extended to return to court in February.
