The Henty Highway was shut for more than an hour from 11am as emergency services attended.
Horsham Highway Patrol Leading Senior Constable Heath Martin said a Dimboola man, 82, was driving a Toyota Hilux dual-cab ute north along Old Hamilton Road.
"When turning right onto the Henty Highway, he has failed to give way to a south-bound Mazda tip-truck," he said.
"Both men were trapped in their vehicles for some time and, with the assistance of State Emergency Service crews, were removed.
"Both were taken by ambulance to the Wimmera Base Hospital."
A Horsham man, 54, was driving the Mazda tip-truck.
Ambulance Victoria spokesman John Mullen said the Horsham man suffered leg injuries and a head laceration.
Paramedics conveyed the man to Horsham's Wimmera Base Hospital in a serious but stable condition.
Mr Mullen said the Dimboola man was also taken to hospital by ambulance in a stable condition, but was unsure of his injuries.
