The Dellar Medal was won by Harrow-Balmoral's Nick Pekin and Edenhope-Apsley duo Jak Ryan and Lachlan Middleton on Monday night.
In one of the most exciting vote counts in recent memory, just two votes separated the top seven players.
The trio won with 19 votes, while Rupanyup's Elliot Kelly finished fourth with 18, and Jeparit-Rainbow's Daniel Batson, Harrow-Balmoral's Eric Guthrie and Natimuk United's Sam Anson finished in equal fifth with 17 each.
The only other three-way tie in the league's history was in 2002, when Noradjuha-Quantong's Marcus Anson, Kalkee's Scott Bachelor and Rupanyup's Jarrod Wood shared the top honour.
Pekin said he was surprised to win back-to-back Dellar Medals.
"I'm totally shocked to be honest, just because of the calibre of players we've got down there at Harrow-Balmoral," Pekin said.
"With Eric Guthrie, we were both stealing votes off each other at the end.
"I'm very surprised, but ... I'm very honoured and very proud to be standing here with Jak, and Middsy (Middleton) as well."
Ryan said it was particularly special to share the award with a teammate.
The two would have combined on many occasions this season, with Ryan winning the ruck tap down to the young midfielder.
"I couldn't tap it to a better bloke," Ryan said. "He's worked so hard for it.
"Even in the finals campaign he had a groin injury and he pushed through it.
"All the boys call him the horse, and he's a bloody work horse every day.
"It's not only special to win it with Lachie, but with Edenhope-Apsley as well. It's my home town, so (it was great) to bring it home for them."
Middleton, 19, was unable to attend the awards night as he is living and working in Geelong.
The winning trio were trailing for a large portion of the vote count, with Swifts' coach Scott Carey holding a one-vote lead ahead of Pekin and Ryan after round 13. Carey however succumbed to an ankle injury in round 14, and didn't add to his 16 votes for the remainder of the count.
Middleton charged home in the final five rounds, securing eight votes, while Pekin and Ryan shared four each to finish on 19.
Pekin wanted to thank his family for their support.
"It's a bit of a reward for Sarah my wife for putting up with a hell of a lot," he said.
Ryan too thanked his partner and the club.
"My missus for putting up with me complaining about sore shins and corkies, props to her," he said.
In the reserves, Noradjuha-Quantong ruckman Jordan McDonald claimed the Richie Dunlop memorial award with 20 votes, while there was a three-way tie for the runners-up.
Kalkee's Brendan Hobbs, Harrow-Balmoral's Quentin Willmott and Jeparit-Rainbow's Steven Murphy all tied on 18 votes.
McDonald said it was a total surprise, particularly as he was six votes behind Hobbs after round eight.
"Very unexpected, I thought Hobbsy would take it out, especially leading with 14 votes after round eight," McDonald said.
"I've been playing in the ruck the last couple of years and it's good to get into the game nice and early, throw a bit of my weight around and try to win it for the team."
