HORSHAM Highway Patrol Senior Constable Jade Spence said the incident happened at the intersection of the Western Highway and Crutes Track, less than five kilometres west of Dadswells Bridge.
The area is a 100km/h zone.
"It looks like the car towing the caravan was driving the speed limit and the truck travelling behind it got impatient and attempted to overtake the car," she said.
"As the truck was overtaking, the semi-trailer was wobbly and smashed into the front of the car. This caused the car to run into the guardrail.
"The second truck which was behind both vehicles has then rear-ended the caravan."
Senior Constable Spence said a 62-year-old woman, who was the passenger of the car, and a 51-year-old man, who was the driver of the rear truck, were taken to Horsham's Wimmera Base Hospital in stable condition.
She said police would continue investigations into the crash.
The incident comes less than a month after an American woman died in a car and truck crash on the Western Highway at Dadswells Bridge.

TWO people have been taken to hospital in a stable condition after a crash involving two semi-trailer trucks and car towing a caravan on the Western Highway.
Emergency services crews were called to the incident near Dadswells Bridge at about 3pm on Thursday.
A Horsham Highway Patrol officer said the incident involved two trucks and a caravan.
An Ambulance Victoria spokeswoman said two people to Horsham's Wimmera Base Hospital in a stable condition.
