Delahunty contributed for the Lions with 10 touches and 20 hit-outs, while his teammate Greg Clark won the Simpson Medal for his 26 disposals and two goals.
The flag win tops off what has been a strong year for Delahunty, who was also named as starting ruck in the WAFL Team of the Year.
The news wasn't as good for former Horsham footballer Chris Curran over in the South Australian National Football League grand final.
Glenelg, partly led by Curran, was defeated to the tune of 67-points by Woodville-West Torrens.
The Tigers won the minor premiership earlier this year, but unfortunately never really got going against the Eagles.
Curran, who was named as captain of the SANFL Team of the Year finished with 17 disposals for his side.
