Investigators have been told a cement truck travelling north towards the intersection with Williams Roads collided into an SUV, which in turn collided with a utility.
The female driver of the SUV died at the scene. Three other female occupants of the vehicle were taken to the hospital with serious injuries.
The driver of the utility vehicle sustained minor injuries and was taken to hospital.
The driver of the truck, a 52-year-old Horsham man, was assisting police with their inquiries.
Investigators are urging anyone with dashcam footage of the incident to come forward.

EARLIER, 3pm: POLICE, ambulance and CFA are on the scene of a traffic accident at Stawell Road, Horsham on Friday.
The incident occurred outside the Glynlea Motel on the west bound side of the road.
The Wimmera Mail-Times understands there could be up to two people trapped in an incident that has been described as between a truck and up to two to three cars.
The incident has been listed as under control by CFA.
SES has been called to the scene.
Traffic is being redirected away from the scene - the road is blocked.
It appears a concrete truck and a vehicle have collided.
