Facing off against the Perth Wildcats in a best of three games grand final series, the 36ers lost game one before rebounding in game two on Friday night to level the series.
The last match was played yesterday afternoon in Perth, with the Wildcats dominating in a 93-59 win.
Creek scored six points and grabbed four rebounds in the decider.
Wildcat Jermaine Beal was named the series MVP.
Creek took to social media after the loss to express disappointment, but said the team would come back stronger next year.
"The pain of losing a grand final, coming up just a little short on the day, absolutely kills," he said.
"We achieved amazing things this year sadly we didn't get what we came here to get.
"We will be ready, determined and accepting of any and every challenge the 2014-15 NBL season has to offer."
